Eligible hourly retirees or surviving spouses retired prior to 10-1-03 will receive a lump sum payment in December 2006. Payments will equal $800 for eligible retirees, provided they do not have an overpayment under the Life and Disability Benefits Program. If there is an overpayment, then the hourly retireee will be eligible to receive only 65% of the $800 (or $520). Eligible surviving spouses will receive 65% amount that would have been payable to the deseased hourly retiree.Payments will be dated 12-15-05.

How do we establish a Personal Identification Number (PIN) with Fidelity?

• If you have a Personal Savings Plan (PSP) and have already established a PIN, you may use the same PIN to access the Fidelity system.
• To set up a new PIN, call the GM Benefits & Services Center at 1-800-489-4646.
• Enter your Social Security Number (SSN) when prompted. Enter your date of birth when prompted. Enter your ZIP code when prompted.
• Input a 6-12 digit PIN. You will be asked to enter this SAME PIN twice for confirmation.
• Finally, you will be asked to set up one of these five reminder questions: First name of your best friend from high school; favorite restaurant; first pet; model of your first car; or name of street where you grew up. If you forget your PIN, your answer to the question you select will serve as verification and allow you to re-establish your PIN.

NOTE: Almost two years after GM switched from EDS to Fidelity as our benefits administrator, members are still discovering their families have been dropped from their health care insurance, bills are rejected, retirees are running into problems, SUB pay checks have been delayed because of improper coding, people are being randomly switched from one insurance carrier to another...it would be a good idea to call the National Benefits Center or check the BGM Benefits web site and make sure all your coverage is in order BEFORE you need it!
